Requirements for Enrolment

~Proof of Birth (either birth certificate or passport)

~If child is born overseas, we must have date that the child arrived in Australia together with a valid visa/passport.

~Imunisation certificate. This can be obtained from Immuniation Registry or from Medicare Office.

~Medicare Number, Health insurance and Ambulance member number.

~Family Doctor's details.

~At least 2 emergency contacts.

~Parent's work details.
